BILLING POLICY

A. Patients are responsible for keeping a valid credit card on file and understand their card will be charged the day of their appointment regardless of the actual appointment time.

 B. Insurance Benefits or Self-Pay:

We are contracted with Anthem, Aetna, Optum/United Health Care, Cigna/Evernorth, Magellan, Blue Shield, Humana, Medicare, Triwest and IEHP (through Parra Counseling and Wellness Center). We are adding more so please call for updated info. When you book an appointment, you book as a private or self-pay, and an intake packet will be emailed to you. If you have insurance, completing your intake paperwork and submitting it early will allow us to confirm your benefits and advise you of your out-of-pocket charges as soon as possible. The card you provide to be kept on file will be charged your portion of copay or deductible or full pay on the morning of your appointment unless you cancel 24 hours in advance.

 C. Your time slot is reserved for you. We do not over/double book, so it is important you cancel at least 24 hours before your appointment time. No-shows may be charged the full rate. We understand emergencies happen if this is the case please contact us.
