New Client Pre-Screening & Insurance Verification

To help us determine if our services are the right fit and to verify your insurance benefits, all new clients are asked to complete a brief pre-screening form before scheduling an appointment.

Once the form is submitted, our team will review the information and contact your insurance provider to confirm your coverage, copay, deductible, and any out-of-pocket responsibilities.

If your insurance is accepted and services are covered, we will reach out to schedule your appointment.

Credit Card Authorization

After your insurance benefits are verified, you will receive a secure credit card authorization form via email. This form must be completed before your first appointment can be scheduled.

If your plan includes a copay, deductible, or coinsurance, the amount you are responsible for will be charged to the card on file on the morning of your appointment.

For your security and privacy:

  • Our office does not have access to your full credit card information

  • All payment information is processed and securely stored through Square, a trusted and encrypted payment processor
